Pure luck, I assure you.
Got ridiculously lucky in the KG entry contest when he was only Expert with the sword - survived the bloodbath inside and outside Kings Landing in the recent war with Prince Andry and got to be Lord Commander more through the fact there was only 3 of us left at that point!

The Regency's another story - the lesson is "Don't laugh & be smug at the Smith as he struggles to control the realm"

But you're right - he was born the son of a blacksmith, became a rather dubious hedge knight and the rest just happened.
